Journey to Independence Program Receives Funding from Boomer Esiason Foundation
Randy Hunt, MD, Brandi Middour-Oxler, DNP, and Shaina Blair, LCSW received a nice grant from the Boomer Esiason Foundation that is supporting the transition program. Specifically, the funding will support implementation of the CF transition program (Journey to Independence) through support for the Adult Care Transition Coordinator, Mental Health Coordinator, and the development and implementation of comprehensive transplant education programming. The team anticipates that this initial investment will allow them to create and fully implement the new transplant curriculum into the transition program, and position it for sustainability beyond the grant period.
